The Central West LHIN plans, funds and integrates local health care services in Brampton, Caledon, Dufferin County, Malton, North Etobicoke-Rexdale and Woodbridge.
We work together with hospitals, community-based services, Long-Term Care facilities, physicians, family health teams, and community health centres, to improve access to health care services for residents in our communities.

Central West LHIN hosts 3rd Annual Chronic Disease Prevention and Management (CDPM) Conference
The Central West LHIN is proud to have recently hosted the 3rd Annual CDPM Conference, at which over 250 health care professionals came together to learn more about the treatment and management of chronic diseases, in order to improve patient–centred care.
Featuring keynote speaker Dr. Barry Simon, Assistant Professor of Psychiatry at the University of Toronto and Psychiatrist at the Leadership Sinai Centre for Diabetes, the conference supports work of the LHIN and local Health Service Providers in keeping with the Integrated Health Services Plan (IHSP 3) and its focus to improve access to CDPM programs, with an emphasis on diabetes.
The conference provided health care professionals with important and meaningful information regarding the fundamentals of patient empowerment, and how to effectively care for adults living with chronic disease.
