Core Action Groups

Since its inception in 2006 the Central West LHIN has created several Core Action Groups. These groups, made up of health care professionals, health service providers and LHIN representatives, take an active role in leading health system planning and development in their specialized areas. The Central West LHIN is committed to developing community based primary care health services. This was identified as a priority in both our Health Systems Plan and second Integrated Health Service Plan, and is a focus for us in the ongoing development of local health care. To continue building on this process the Central West LHIN has established the Bolton and Area Community Health Work Group and the Shelburne Community Health Task Force. These groups will provide advice to the Central West LHIN  about the development of local community based care. For more information on these two groups please select a link below.

 

A comprehensive study of rehabilitation services in Central West was undertaken.  The study included a review of practices locally, data collection and analysis, stakeholder and academic interviews,  an international literature search, and a day-long forum.  The final report entitled Smoother Transitions Better Outcomes Report, Building a Framework to Rethink Rehab was received in August 2010.   The Central West LHIN project entitled the Rehabilitation Framework Project began in November 2010 with the creation of the Rehabilitation Leadership Team.
