Healthy Communities Initiative

Since 2015, the Central West Local Health Integration Network (LHIN), Region of Peel and William Osler Health System – in collaboration with the City of Brampton, local school boards and a diverse group of engaged community partners – have been working to address population health through a unique and innovative enterprise known as the Healthy Communities Initiative (HCI).

HCI’s vision is a healthy community in which all residents have the environments and opportunities to achieve better health and wellness. Achieving this vision and addressing the underlying causes of chronic disease requires the support of an entire community.

Healthy Communities Initiative - 5-2-1-0 Logo 

HCI is informed by a simple prescription for promoting and maintaining good health: 5-2-1-0. Living 5-2-1-0 means eating at least five servings of fruits and vegetables a day, limiting screen time to two hours per day, being physically active for at least one hour per day, and consuming zero sugar-sweetened beverages throughout the day.

5-2-1-0 is an internationally-recognized construct that has been adapted by many health promotion initiatives. By following this simple prescription for change, we can reduce our risk of acquiring chronic diseases such as diabetes and cardiovascular disease. Positive health behaviours such as eating healthy foods and being physically active are also a protective factors for our mental wellbeing. Through HCI, we hope to enable our whole community to live 5-2-1-0 everyday!

HCI Settings

Some of HCI’s greatest momentum has come from the school setting. There are 26 HCI schools in Brampton, from both the Peel District School Board and Dufferin-Peel Catholic District School Board. All of our HCI schools are committed to creating school environments that are supportive of healthy choices.

Faith Settings

Places of worship are important community destinations where people gather and seek guidance and advice on many matters, including those related to overall wellbeing and health. HCI is pleased to partner with many faith settings who are representative of our diverse community, and who all share a common objective of enabling health and wellbeing for their congregants and community.

Built Environment
