Health System Plan
An Innovative Roadmap to Increase Access to Health Services


BACKGROUND
The Central West LHIN - with input from the community, community organizations, service groups and health service providers - has been building a detailed system vision and plan for the development of a comprehensive and integrated system of community and hospital-based health services.

On Wednesday, May 28th, the Board of the Central West LHIN received the Health System Plan report from PricewaterhouseCoopers.


HEALTH SYSTEM PLAN REPORT OVERVIEW

The Health System Plan final draft report outlines a number of strategic goals for a person-centred, community-based health system, including reinforcing the expectation that people improve their health by making healthy lifestyle choices and that health service providers share accountability for improved health care outcomes.  

No one organization or sector can individually   address all the public’s health care needs.  Health service providers will work together in a new and fundamentally different way, as though there is one fully integrated organization with one common goal – the improvement of the health status of the residents of the Central West LHIN. 

Community-based health services are viewed as the first point of contact for residents of the Central West LHIN, with the goal to provide more services which supports health and well-being closer to where people live. 

The report recommends a fully integrated hospital system which provides a comprehensive range of services to meet the needs of Central West LHIN residents and support the community-based service providers.


RECENT COMMUNITY ENGAGEMENTS

The Central West LHIN hosted public sessions geared at sharing our report findings with our various communities to seek their feedback.
