Central West LHIN Aging at Home Strategy
Aging at Home Year 3
The Central West LHIN is now requesting submissions for Aging at Home Year Three Projects. Please find below the guidelines and templates to help in the development of your submission. The Deadline for Submissions is November 30, 2009 by 12:00 noon.
Background
On August 28th, 2007 the Ministry of Health and Long-Term Care announced an investment of $702 million over three years for an Aging at Home Strategy. This Strategy is aimed at providing seniors and their families and caregivers with an integrated continuum of community-based services to enable them to stay healthy and live more independently in their homes. As a part of the Strategy, it was announced that the Central West LHIN will receive 21.5 million. This exciting initiative will impact the way services are delivered and help provide more equitable access to health care by matching the needs of the local senior population with the appropriate support services.
The Strategy aims to keep seniors healthy by providing a comprehensive mix of services for seniors and their caregivers through traditional services combined with innovative practices. The new funding is targeted to 2 main areas:
1. Increasing the overall supply (mix and quantity) of services (e.g. capacity building) that support seniors to stay healthy and live safely at home with independence and dignity, including:
2. Leveraging change through innovation, such as new preventative and wellness services and partnerships with non-traditional providers.

Detailed Service Plan
The Aging at Home Detailed Service Plan continues work begun with the development of the Aging at Home Directional Plan developed by the Central West LHIN in October, 2007. The Central West LHIN’s Detailed Plan will keep seniors healthy by providing a comprehensive mix of both traditional and innovative services for seniors and their caregivers.

The Directional Plan provides a high-level overview of the draft 2008/2009 directions that the Central West LHIN is pursuing for the Aging at Home Strategy.
Local Aging at Home Initiatives
Interested agencies and organizations are encouraged to submit initiatives that would support the local Aging at Home Directional Plan by filling out the Health System Improvement Pre-proposal form below.
